Getting Started

If you like, you can simply make an appointment.

    You can call me at 512-464-1146, or send me an email. Let me know what times work best for you,
    and how I can best reach you. I can usually respond within one working day. For my office location,
    click on map.

There are a few forms to fill out.

    You’re welcome to download them early and finish them at your leisure. (Don't forget to bring them to
      your appointment!)

If you plan to use insurance, I’m a provider for:

    BCBS, Cigna, Great West, Humana, PHCS, Principal, and Value Options

If you’d like faster confirmation of coverage and your co-pay:

    Give me the following information before your appointment:

         ▪ Insurance company
         ▪ Primary subscriber
   ▪ Insurance ID#
   ▪  Group ID#
   ▪ Your name (if different)
   ▪ " address (as listed)
   ▪ " date of birth

    I usually get benefits info within one business day. (You can also call them directly.)

If you don’t want to use insurance:

    My regular fee is $110 per session. However, due to the state of the economy, I have reduced it to $80.
To discuss fees or any other issue, give me a call and we'll talk.
